Change in policy - only the players in the 15's
and up teams need to fill out a bio form. The new plan is to just put
everyone's name and picture in a Club Directory and only use the bio
information on the web for recruitment purposes.
The forms on this page are used to enter your
biographical
information so that it may appear in the CVC web pages and possibly the
player team sheets. The bio forms will NOT be filled out at the CVC Banquet as
in previous years. The best way to fill out any of the three bio forms is to
copy and paste your information from last year and then
add to it. Better yet, just update last years form. Here is a good example
of a bio.
The preferred way to do the bio is to
download your old bio, update it, and then send
it back to the
webmaster. The next slightly less preferred way is to download either the Microsoft Word-based form or the
text-based form, fill it out, and then email it back
to the webmaster. If
you are unable to work with an HTML page or either of these documents, then the least desired
approach is to fill out the following web form. Please contact the
webmaster if your
information does not appear on the web pages within a few days or you would
like to make updates/corrections.
